To complete the “initial†Charitable Trust (CT) Form, bring the following documents and information:
(1) Copy of your current bylaws signed by your secretary. If you need a copy or need to purchase bylaws (there is a $1.00 district fee.) If your bylaws are 4+ years old you need to update them TODAY!
(2) $25.00 initial fee payable to:Â Department of Justice
(3) Roster of your 2012/2013 officers. (If we do not have one)
(4)Â PLEASE RSVP, so that we at Tenth District can do our part to have your pre-filled
Charitable Trust (CT) Forms ready.
